Athletes under 26 and parents of athletes under 18 years of age must confirm that they have reviewed the
Government of Ontario Concussion Awareness Resources below and completed the Skate Ontario’s Concussion Code of Conduct for Athletes and Parents/Guardians prior to registration and/or on the first day of skating.

Please ensure that you review the resources appropriate to your family’s situation (each skater must have a waiver completed after reviewing these resources prior to skating), as well as Skate Ontario’s Concussion Code of Conduct.

Government of Ontario Concussion Awareness Resources – Rowan’s Law E-booklet:
• Ages 10 and Under
• Ages 11-14
• Ages 15 and Up

Once reviewing the resources above, click here to fill out the Skate Ontario Waiver. Please send a copy of your completed waiver to tilburyfsc@gmail.com or bring a printed copy with you the first day of skating for each skater.
